How to request an IRS Form W-9

OSSDS will provide a completed IRS Form W-9 at your request.

 Instructions

1. Access the OSSDS customer portal by clicking on this link: https://ossds.atlassian.net/servicedesk/customer/portals

2. Click on “Operations”

3. Click on “Accounting”

4. Summary will be “Supply [insert your business name] with your IRS Form W-9”

5. Description will be your company information

6. Due date will be two business days

7. Set Share with to “Share with OSSDS”
